A 61 year old man with involuntary movements, fever and shortness of breath.

Chief complaints

A 61 year old man who is a resident of Ramannapet presented to the OPD complaining of shortness of breath, fever, dry cough since 3 days,and involuntary movements in the right hand since 3 years. He is also experiencing a burning sensation in the chest. 

History of present illness

Patient was asymptomatic few months back. Then he developed  which was relieved on medication. Since 3 days, he has burning sensation in  and difficulty in breathing in supine position.

No diabetes
No hypertension
No CAD
no asthma
No syphilis
No epilepsy

Family history

No significant family history

Allergies

No allergies

PHYSICAL EXAMINATION

Non coherent
Non cooperative
Well built
Hydrated
Pallor is absent
No icterus
No cyanosis
No clubbing of fingers
No lymphadenopathy
Pedal edema absent
